V. veröffentlicht in ihrer Leitlinie „Gute Substitutionspraxis“, dass eine Substitution bzw. ein Austausch von Antiasthmatika und pulmonal angewendeten Arzneimitteln kritisch sein kann.<sup>1</sup> Durch eine Substitution können die Adhärenz und der Therapieerfolg gefährdet werden. Dieser Gesichtspunkt sollte in der Apotheke bei einem möglichen Austausch, etwa aufgrund von Rabattverträgen oder einem grundsätzlich möglichen Aut-idem-Austausch, berücksichtigt werden. Im Fokus sollte die gleichbleibende Therapie des Patienten stehen; um diese zu unterstützen, ist es auch möglich, den Arzt um eine Verordnung mit Aut-idem-Kreuz zu bitten.
