Gut gemeint: Das Entlass­management soll seit Oktober 2017 die Versorgung der Patienten nach einem Kranken­haus­aufenthalt bis zur Übernahme durch den Haus­arzt und die Haus­apotheke sicher­stellen.</p> <p>Leider konnte man sich nicht auf eine über­sichtliche und einfache Verordnungs­vorschrift einigen, sodass sowohl eine <strong>Ergänzung zum Rahmen­vertrag</strong> als Grundlage für alle GKV-Kassen als auch eine neue <strong>Ergänzungs­vereinbarung mit den Ersatz­kassen</strong> beschlossen wurden.
