B. aufgrund einer engen therapeutischen Breite). </p> <p> Mittlerweile sind eine Reihe von Wirkstoffen definiert worden, bei denen ein Austausch generell untersagt ist. Die Wirkstoffe der Substitutionsausschlussliste sind in Teil B der Anlage VII zur Arzneimittel-Richtlinie mit den zugehörigen Darreichungsformen aufgeführt. </p> <p> Das generelle Austauschverbot bedeutet, dass Apotheken ein Arzneimittel mit einem Wirkstoff der Substitutionsausschlussliste nicht austauschen dürfen – auch nicht im Notdienst oder wenn es ggf. Rabattverträge für wirkstoffgleiche Arzneimittel gibt.
