Auf diesem werden zu möglichst allen Arzneimitteln, die der Patient einnimmt, der Wirkstoff, die Dosierung, der Einnahmegrund und sonstige Hinweise zur Einnahme aufgeführt. </p> <p> Um zu erfahren, ob sich dieser bis heute etabliert hat, haben wir unseren Lesern die folgende Frage gestellt: </p> <p> <b>Wie vielen Patienten liegt Ihres Wissens ein Medikationsplan oder eine andere schriftliche Dosierungsanweisung vor?</b> </p> </font> <img class="image-topthema" src="https://www.deutschesapothekenportal.de/fileadmin/news/newsletter/2019q4/newsletter_2019-10-20_g1.png" width="660" vspace="10"> <font style="font-family:Helvetica,Arial,sans-serif;font-size:14px;line-height:20px;color:#555555;" size="1" face="Helvetica,Arial,sans-serif;" color="#555555"> <small> Abb.: DAP Kurzumfrage, Quelle: DAP Networks<br><br></small> </font> <font style="font-family:Helvetica,Arial,sans-serif;font-size:14px;line-height:20px;color:#555555;" size="1" face="Helvetica,Arial,sans-serif;" color="#555555"> <p> Das Ergebnis: Nur gut sieben Prozent der Teilnehmer gaben an, dass sehr vielen ihrer Patienten ein Medikationsplan bzw. eine schriftliche Dosierungsanweisung vorliegt. Knapp ein Drittel (29,3 %) der Teilnehmer berichtete, dass viele Patienten über einen Medikationsplan verfügen. Mit 34,9 Prozent gaben die meisten Teilnehmer der Umfrage an, dass etwa die Hälfte der Patienten einen Medikationsplan bzw. eine schriftliche Dosierungsanweisung besitzt. 18,9 Prozent der Umfrageteilnehmer ist das Vorliegen eines Medikationsplanes bei wenigen Patienten, 9,6 Prozent der Teilnehmer bei sehr wenigen Patienten bekannt. </p> <p> Das Ergebnis zeigt, dass auch drei Jahre nach Einführung des Medikationsplans viele Patienten diese Möglichkeit offenbar noch nicht nutzen.
