Erstellt und aktualisiert wird der Plan von einem Arzt, meist dem Hausarzt. Die Apotheke darf den Medikationsplan hingegen nur auf Wunsch des Patienten aktualisieren (z. B. Es bleibt abzuwarten, ob dies in naher Zukunft auch für die Apotheken gilt.
