Das Apothekenpersonal kann eine Substitution aufgrund Pharmazeutischer Bedenken unterbinden, sofern der Anwendungs- und Therapieerfolg gefährdet ist. Im Fokus sollte die gleichbleibende Therapie für den Patienten stehen. In einem solchen Fall empfiehlt DAP, das Rezept mit der Sonder-PZN 02567024 und der entsprechenden Kennziffer zu versehen. Eine handschriftliche Begründung (inklusive Datum und Unterschrift) ist zusätzlich erforderlich.* </p> <p> Wie Pharmazeutische Bedenken korrekt zu dokumentieren sind, zeigt die <a href="https://www.deutschesapothekenportal.de/rezept-retax/dap-retax-arbeitshilfen/pharmazeutische-bedenken/dokumentation-auf-dem-rezept/" target="_blank" style="text-decoration:none;color:#005CA9;">DAP Arbeitshilfe „Pharmazeutische Bedenken – Dokumentation auf dem Rezept“</a>. </p> <p> Der Arzt kann einen Austausch komplett unterbinden, indem er das Aut-idem-Kreuz setzt und vermerkt, dass der Austausch aus medizinisch-therapeutischen Gründen untersagt ist (gilt für Ersatzkassen gemäß vdek-Arzneiversorgungsvertrag).** </p> <p> Findet dennoch ein Austausch auf einen nicht wiederverwendbaren Import statt, so ist der Kunde darüber aufzuklären, dass er den RESPIMAT<sup>®</sup> samt leerer Wirkstoffpatrone entsorgen muss.
