Html: <!-- ------------------------------------------------------------------ --> <table> <tbody> <tr> <th class="force-col" style="font-weight:normal;text-align:left;" valign="top"> <table class="news_first_2" dir="ltr" width="660"> <tbody> <tr> <td> <div style="text-align:right;"> <font style="font-family:Helvetica,Arial,sans-serif;font-size:12px;line-height:21px;color:#777777;" size="1" face="Helvetica,Arial,sans-serif;" color="#777777">Anzeige</font> </div> <h1 style="font-family:Helvetica,Arial,sans-serif;font-size: 16px;text-transform:uppercase;margin-top:0px;"> VIFOR PHARMA INFORMIERT </h1> <h2 style="font-family:Helvetica,Arial,sans-serif;font-size: 16px;color:#005CA9;margin-top:0px;margin-bottom:5px;"> Eisenmangel – ein oft unterschätztes Problem bei chronisch entzündlichen Darmerkrankungen </h2> <font style="font-family:Helvetica,Arial,sans-serif;font-size:14px;line-height:20px;color:#555555;" size="1" face="Helvetica,Arial,sans-serif;" color="#555555"> <p> Patienten mit chronisch entzündlichen Darmerkrankungen (CED) wie Morbus Crohn oder Colitis ulcerosa weisen zu 60 bis 80 Prozent einen Eisenmangel auf und etwa ein Drittel der Betroffenen sogar eine Anämie.<sup>1</sup> </p> <p> Mögliche Symptome sind Erschöpfung<sup>2</sup> und Fatigue<sup>2,3</sup>, kognitive Einschränkungen<sup>2,3</sup>, Kopfschmerzen<sup>2,3</sup>, Schlafstörungen<sup>3</sup>, erhöhte Infektanfälligkeit<sup>2</sup> sowie Störungen der Schleimhautregeneration<sup>2</sup>. </p> <p> Sowohl eine verringerte Eisenzufuhr als auch eine verminderte Resorption, bedingt durch eine geschädigte Darmmucosa, und/oder ein erhöhter Eisenverlust durch gastrointestinale Blutungen können Ursache eines CED-bedingten Eisenmangels sein.<sup>3,4,5</sup> Hinzu kommt, dass bei (chronischen) entzündlichen Prozessen nicht genügend Eisen zur Verfügung gestellt werden kann<sup>4,6</sup>, da die systemische Eisenhomöostase durch proinflammatorische Zytokine negativ beeinflusst wird. </p> <p> Alle CED-Patienten sollten auf das Vorliegen einer Anämie getestet und bei manifester Eisenmangelanämie, um das Befinden der Patienten positiv zu beeinflussen, mit Eisen substituiert werden, so empfiehlt es die Konsensusleitlinie der European Crohn´s and Colitis Organisation (ECCO).<sup>3</sup> Auch die Deutsche Gesellschaft für Gastroenterologie, Verdauungs- und Stoffwechselkrankheiten (DGVS) fordert in ihren Leitlinien zur Colitis ulcerosa<sup>7</sup> sowie zum Morbus Crohn<sup>8</sup> eine Blutbilddiagnostik einschließlich des Eisenhaushalts vorzunehmen. </p> <p> Eine i.v. Eisengabe wird bei klinisch aktiver Erkrankung, Unverträglichkeit von oralem Eisen, bei Hb-Werten < 10 g/dl sowie bei Bedarf für Erythropoese-stimulierende Agentien empfohlen.<sup>3</sup> Die aktuelle S 3-Leitlinie zur Behandlung von Colitis ulcerosa<sup>7</sup> bzw. Morbus Crohn<sup>8</sup> nennen zusätzlich das Nichtansprechen auf orales Eisen<sup>7,8</sup> sowie Patientenpräferenz<sup>7</sup> als weitere Indikationen für eine i.v. Eisengabe. Eisencarboxymaltose erwies sich nach Daten einer Metaanalyse von fünf randomisierten kontrollierten Studien als das effektivste i.v. Eisenpräparat in Bezug auf eine Hb-Normalisierung und einen Hb-Anstieg<sup>9</sup>. Zum Vergleich: Beim Referenzpräparat (Suboxone<sup>®</sup> 8/2 mg) beträgt die Zeit bis zur vollständigen Auflösung 12,5 Minuten.
